Welcome to the Practitioner Hub — a central resource for health care professionals, community leaders and advocates working across the food and health ecosystem. Health Care by Food invites food is medicine practitioners from both the public and private sectors to share their contact information in a comprehensive and inclusive hub designed to foster collaboration, connection and knowledge sharing.
